Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms, ahead of time, you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.

If you already have a copy of Adobe Acrobat Reader, then you may download a form immediately by clicking on the name of the form below.

The PDF file sizes are relatively small and will take only a few minutes to download, depending upon your modem and connection speed. When the file transfer is complete, just click on the filename to launch Acrobat Reader and open the form and print it.

If you do not have a copy of Acrobat Reader, you can get one for free by clicking on the icon ‘Get Acrobat® Reader’. This will take you to Adobe’s website and enable you to download the Acrobat program to your computer.

If you belong to a managed care plan (HMO), please make sure that your primary care physician has sent Dr. Slusher a referral for your appointment. Please bring any insurance ID card you may have including Medicare and Medicaid.

Non-covered charges or co-payments are due at the time of service. Medicare and some other insurance plans do not pay for refractions (the test to determine your glasses prescription).

If there is a question about insurance coverage for your visit, please let us work on it before you arrive.

Things to bring to your appointment:

  • Current glasses/contact lenses
  • Current eye drops and/or ointment
  • Current list of medications
  • Insurance card
  • Picture I.D.

If you must cancel or reschedule your appointment, please let us know as soon as possible.

We accept all credit cards, personal checks and cash.
